For Ages 13–18

Our Kids Direct Primary Care membership is designed to provide teens with convenient, personalized healthcare in a setting that prioritizes time, education, and prevention.

During the teenage years, healthcare needs often expand to include sports physicals, illness care, mental health support, and guidance through important stages of growth and development. Our Direct Primary Care model allows for longer visits and direct access to your provider so families can feel confident their child is receiving thoughtful, comprehensive care.

Membership includes routine primary care services such as sick visits, wellness visits, school and sports physicals, and ongoing support for common health concerns.

Younger children may be accepted on a case-by-case basis, depending on the needs of the family and current membership availability. If you are interested in care for a child under the age of 13, we encourage you to reach out to discuss your family’s needs.

For Ages 18+

Our Adult Direct Primary Care membership is designed to provide personalized, accessible healthcare focused on prevention, long-term wellness, and meaningful relationships between patients and their provider.

In today’s traditional healthcare system, appointments can feel rushed and access to your provider can be limited. Direct Primary Care removes those barriers, allowing for longer visits, direct communication, and care that focuses on the whole person rather than just treating symptoms.

 For Two Adults and Up to Four Children

Our Family Direct Primary Care membership is designed to provide convenient, personalized healthcare for the entire household. By caring for multiple members of the same family, we are able to support your family’s health with a more connected and comprehensive approach.

Direct Primary Care allows families to have reliable access to their provider without the barriers often created by traditional insurance-based healthcare. With longer visits, direct communication, and a focus on prevention, families can feel confident that their healthcare needs are being addressed with time and attention.

This membership supports routine care for both adults and children, including sick visits, wellness visits, and guidance to help families stay healthy through every stage of life.

Family memberships include coverage for two adults and up to four children, creating a simple and affordable option for families seeking consistent, relationship-based care.
